Where hospital overtime comes from
Record a reason with every claim, because the pattern of reasons tells the nursing superintendent more than the total does. In hospital paperwork, write 'operation theatre' in full on overtime forms and registers: OT means overtime to HR and the theatre to everyone else, and the two get confused in month-end sheets.
- Double duty when a reliever does not turn up
- Operation theatre lists that run past the shift
- Handover delays after a code blue or an admission rush
- Monsoon and winter surges when every bed is full
- Fire drills, audits and training held after a shift
The legal rate and consent
Under the OSH Code, ordinary daily work is capped at eight hours, and work beyond the prescribed hours is overtime, paid at twice the ordinary wage rate, with the worker's consent. See the working hours and overtime rules. Hospitals sometimes pay double duty as a fixed amount, such as a few hundred rupees a shift; that falls short whenever the fixed amount works out lower than twice the hourly wage for the hours worked. Record consent for overtime, particularly for night double duty, and keep the multipliers for working days, weekly offs and holidays at 2 or more.
Pay or comp-off
Many nurses prefer a day off to cash, especially after a run of nights. ZeniaHR's overtime policy can pay overtime, credit comp-off or let the employee choose, with 4 hours earning half a day and 8 hours a full day. Overtime is counted after shift end or beyond full-day hours, with a minimum of 30 minutes and rounding down to 15 minutes. A nurse who stays 20 minutes to finish handover notes creates no claim, while one who stays two hours after a code blue does.
Worked example: double duty for a GDA
Rakesh is a GDA with a monthly gross of ₹18,200, and the hospital uses a gross day rate of ₹700 (₹18,200 over 26 days). One evening he stays 3 hours 40 minutes after his shift to cover a colleague who called in sick. Rounded down to 15 minutes, 3 hours 30 minutes count, which is 3.5 hours. His hourly rate is ₹700 divided by 8, or ₹87.50, so at a multiplier of 2 the overtime is ₹87.50 x 3.5 x 2 = ₹612.50. Later that month he works a full 8-hour shift on his weekly off and chooses comp-off instead, which credits one full day.
Approval and monthly caps
ZeniaHR detects overtime every night from punches, so nobody fills a form after a tiring shift. The ward sister approves or rejects each claim, then HR does, and HR can bulk approve at month end once reasons are checked. Set a monthly cap per person, because a nurse on her tenth double duty of the month is a patient safety risk. Approved overtime becomes a payroll earning at the gross day rate divided by 8, times the hours, times the multiplier, or a comp-off credit.
How to set it up in ZeniaHR
- Switch on the overtime policy and choose whether overtime counts after shift end or beyond full-day hours for rostered staff.
- Keep the 30-minute minimum with rounding down to 15 minutes, so short handover delays do not create claims.
- Set the multipliers for working days, weekly offs and holidays at 2 or higher, in line with the legal rate.
- Choose payout as pay, comp-off or employee's choice, and set a monthly cap on overtime hours per person.
- Require approval so ward sisters approve first and HR second, and review overtime by department every month.
- Tell staff they can check their own overtime in My Self-Service and on the mobile app.

Is overtime paid to nurses in private hospitals?
Yes. The OSH Code treats hours beyond the prescribed limit as overtime, payable at twice the ordinary wage rate and worked only with the worker's consent. That covers nurses and support staff who are workers under the law. Many hospitals also offer comp-off instead of cash when the nurse prefers rest; record that choice claim by claim.
How is overtime calculated for hospital staff?
Take the gross day rate, divide by 8 for an hourly rate, then multiply by the overtime hours and the multiplier, which should be 2 or more. A GDA with a ₹700 day rate who works 3.5 extra hours earns ₹87.50 x 3.5 x 2 = ₹612.50. Hours are counted after shift end and rounded down to 15 minutes.
Is double duty the same as overtime?
Double duty is a familiar form of overtime in hospitals: a nurse or GDA works a second shift straight after their own when a reliever is absent. Hours beyond the prescribed limit are overtime, paid at twice the ordinary wage rate with consent. A flat double duty allowance is acceptable only if it pays at least that much, and many hospitals also let staff pick comp-off.
Can a hospital give comp-off instead of overtime pay?
Many hospitals do, when the employee prefers time off and the choice is recorded. ZeniaHR's overtime payout can be set to pay, comp-off or the employee's choice, and comp-off is credited at half a day for 4 hours and a full day for 8. Keep a cash option available, since the OSH Code pays overtime at twice the ordinary wage rate.
