Policy acknowledgement form

A policy acknowledgement form records that an employee has received, read and understood a company policy, such as the code of conduct, the POSH policy, the IT and data security policy or the leave policy. It names the policy, its version and effective date, and carries the employee's signed declaration. When a policy is later enforced in a warning or an inquiry, this form is the proof that the employee knew the rule, which is why it matters as much as the policy itself.

When to use it: Collect it at joining for every core policy, and again from all employees whenever a policy is introduced or materially revised. HR circulates the policy, gives time to read it and answers questions, and each employee signs. HR countersigns and files the form in the personal file or keeps a signed register for shop-floor staff.

What to include

Policy name, version and date

Name each policy with its version and effective date. When a policy changes, the company must be able to show which version an employee acknowledged, especially if a breach happened around the time of the change.

A language the employee understands

Record the language in which the policy was given. An acknowledgement of an English policy by a shop-floor worker who reads only Hindi or Gujarati carries little weight.

Chance to ask questions

A line confirming the employee could ask questions, with the person who answered, shows the acknowledgement was informed and not a signature collected in a hurry.

POSH contact details

Every workplace with 10 or more employees must constitute an Internal Committee under the POSH Act. The acknowledgement for the POSH policy should give the committee's contact details so employees know where to go.

Re-acknowledge on revision

Collect fresh acknowledgements whenever a policy changes materially. An old form for an earlier version does not prove the employee knew the new rule.
