Maternity leave application format

A maternity leave application is the letter a woman employee writes to HR and her reporting manager to ask for paid maternity leave around the birth of her child. It states the expected date of delivery from a doctor's certificate, the date she wants the leave to start, the total period requested and how her work will be handed over. Under the Code on Social Security, eligible women get up to 26 weeks of paid maternity leave, so the application is a notice of an entitlement, not a request for a favour.

When to use it: Submit it several weeks before the planned start of leave, with a medical certificate showing the expected date of delivery, so the team can plan cover and HR can confirm eligibility and pay. Address it to HR with a copy to the reporting manager. HR confirms the leave dates and pay in writing.

Doctor's certificate with the due date

The expected date of delivery from a doctor anchors the leave dates. Attach the certificate, and update HR with the hospital's certificate after delivery if the dates move.

Start date and return date

Give the date the leave starts and the planned return date. The employee chooses the start date within the rules, and the team uses both dates to plan cover.

Handover without pressure

A handover plan helps the team, but the employee should not be pressed to postpone leave or keep working from the hospital. Managers should plan cover as soon as the notice arrives.
