Performance management for diagnostic labs

A lab's quality shows up in numbers it already records: sample rejections, recollections, turnaround time, QC failures and amended reports. Good lab performance management picks the few that each role controls, weights them so speed never beats quality, and reviews them monthly for field and front office staff and quarterly for technicians and pathologists.

KRAs that each lab role controls

Pick measures the person can move with their own work. A phlebotomist controls haemolysis and labelling, not the analyser's downtime, and a technician controls QC compliance, not the number of samples the centres send. Goals outside a person's control only teach them that the review is unfair.

Weights that do not backfire

A home collection executive measured only on visits per day will rush and bring back haemolysed samples. Balance speed with quality: for example visits 30, on-time arrival 30, sample rejections 25 and patient feedback 15. In ZeniaHR, goals are typed as KPI, KRA, OKR, competency or task with a weight, and weighted progress is shown per employee. The underlying numbers come from your LIS and booking system; decide who pulls them each month, since ZeniaHR does not read those systems.

Worked example: a phlebotomist's month

Priya's monthly goals are a rejection rate under 1 percent (weight 50), no more than 5 recollection requests (weight 30) and no patient complaints (weight 20). She collected 1,200 samples with 9 rejections, a rate of 0.75 percent, so she meets the first goal. She had 6 recollection requests, missing the second, and no complaints. Scoring each goal as met or not met, her weighted score is 50 + 0 + 20 = 70 out of 100, and the conversation with her supervisor is about labelling and tube checks, not speed.

Review rhythm and recognition

Review field and front office staff monthly, since their incentives already run monthly. Review technicians and pathologists quarterly, with an annual cycle for increments and a probation cycle for new joiners. Between reviews, praise does real work: a Problem solver badge for the technician who fixed a QC failure at midnight, counted on the monthly leaderboard, is noticed across the lab. Check that recognition reaches the collection centres too, not just people at the main lab.

Typical KRAs are the sample rejection rate for haemolysis, clotting or wrong tubes, the number of recollection requests, patient complaints and feedback, and for home collection staff, arrival within the booked slot. Weight quality measures at least as high as volume, so speed never comes at the cost of a usable sample.

How is lab technician performance measured?

Through turnaround time against the lab's targets, internal QC compliance, EQAS results for the section, errors caught at validation and training completion. These come from the LIS and quality records. Review them quarterly with the section head and link the annual cycle to increments.

How often should labs review staff performance?
